You will need to:
- Fill out this form
- Enclose the original cash register receipt dated between 1/1 and 12/31 with the price circled
- Cut out the UPC symbol from the product package
- Have it postmarked by 1/31/2013
One rebate per rebate form. Offer cannot be combined with other rebate offers, manufacturer’s coupon, pre-packaged bonus bags or dollar off bags. There is a $2 off coupon in this Sunday’s paper but unfortunately it cannot be used. Wah.
I bought my Science Diet Dog Food at PetSmart for the small bag at $10.99 plus tax. You may be able to get a similar deal and then get a full rebate and pay nothing for the bag! Now, that’s what I like!
